Necessary for New patients looking to establish Redefined Direct Care and we have NOT already cared for you in the past.

  • $180 per Adult (one-time fee)

  • $100 per Child (one-time fee)

  • 45-60 minute appointment

  • Full review of your medical and surgical history  

  • Physical exam  

  • Lab Orders

  • Personalized preventive health & wellness treatment plan

  • Onboarding for ongoing care through our Direct Primary Care plan.

Step 2: DIRECT PRIMARY CARE PRICING

  • Individual: $115
  • Couple: $207
  • College Student Discount: $65
  • Child (6y-18y): $35 (with adult)
  • Membership is designed for standard primary care needs.
    • Additional visits beyond this scope may incur a small fee.
  • Annual comprehensive exam
  • Routine follow-up and Problem Visits
  • Routine lab monitoring and supplies necessary for Standard Primary care needs. 
    • Additional labs beyond this scope may incur a small fee.
