Step 1: Establish Care

Establishing care helps us to understand your unique health goals and create a plan that fits — whether it’s for Direct Primary Care, Hormones, Peptides, or Medical Weight Loss.

For New patients Establishing Care for Direct Primary Care.

Initial Visit:

  • $180 per adult

Your First Visit Includes:

  • Initial 30–45 minute appointment.
  • Review of past medical history, current symptoms, and goals.
  • Physical exam (if indicated).
  • Initial Comprehensive Labs.
  • Personalized treatment plan.
  • Add-on memberships ARE available.

For New patients Establishing Care for TRT/HRT, Peptides, or Weight Loss.

Initial Visit:

  • $450 per adult
  • $850 per couple 

Your First Visit Includes:

  • Initial 45-60 minute appointment.
  • Review of past medical history, current symptoms, and goals.
  • Physical exam (if indicated).
  • Initial Comprehensive Lab Panel with Advanced Hormone & Vitamins.
  • Personalized treatment plans.
  • Add-on memberships ARE available.

Step 2: Monthly Membership Plans

DIRECT PRIMARY CARE

  • Individual: $115
  • Couple: $207
  • College Student: $65
  • Child (17y or less) w/ Adult: $35
  • Any Current TRT/HRT, Peptide, or MWL patient is eligible for a reduced DPC rate: $95/month

  • Membership is designed for standard primary care needs.
    • Additional visits beyond this scope may incur a small fee.
  • Annual comprehensive exam
  • Routine follow-up and Problem Visits
  • Routine lab monitoring and supplies necessary for Standard Primary care needs. 
    • Additional labs beyond this scope may incur a small fee.
  • Local & Mail Order Pharmacy Options
  • Patient Portal Access
  • Text line with care team.
  • Electronic Referrals
  • Diagnostic Imaging Orders (may require insurance, depending on type of imaging)

Testosterone Replacement Therapy (TRT) &

Hormone Replacement Therapy (HRT)

  • TRT, HRT, Enclomiphene: $180/month
  • Routine In-clinic labs, hormone medications, and supplies necessary for standard TRT/HRT management.
  • Additional labs outside of typical care may incur a fee.
  • Additional medications are available à la carte.

  • A Late Fee will be assessed with 10+ days overdue. Fee is 10% of your outstanding balance.
